For most efficient handling, please include:

1) Rx or referral   2) Patient Demographics & Insurance cards  3) Clinical Documentation  (*IMPORTANT*) 

  • Clinical notes must be comprehensive & should include: 
  • a) first visit note with dates of onset 
  • b) prescribed treatments & therapies with dates 
  • c) the follow-up visit note detailing outcome of treatment/therapy
  • d) applicable medications 
  • e) applicable prior imaging results.

Important Insurance Information

In order to make your visit to our office as smooth as possible, here are a few things to keep in mind.

ELIGIBILITY

It is the patient’s responsibility to provide their current/active insurance at time of scheduling.

PRE-AUTHORIZATION

In order to make your visit to our office as smooth as possible, please keep in mind that it is the patient and referring physician’s responsibility to confirm pre-authorization requirements and obtain the approval that is required for the exam.

We will contact you two business days before your appointment date to reschedule if your insurance carrier has not yet issued the required authorization.

If we have not received the pre-authorization number, please contact your insurance company and/or your referring physician’s office and then provide it to us.
