Important Insurance Information

In order to make your visit to our office as smooth as possible, here are a few things to keep in mind.

ELIGIBILITY

It is the patient’s responsibility to provide their current/active insurance at time of scheduling.

If you have questions or concerns about your insurance eligibility and/or benefits, please call: 239-936-1141

PRE-AUTHORIZATION

Please keep in mind that it is the patient's and the referring physician’s responsibility to confirm pre-authorization requirements and obtain the required approval for the exam.

We will contact you two business days before your appointment date to reschedule if your insurance carrier has not yet issued the required authorization.

If we have not received the pre-authorization number, please contact your insurance company and/or your referring physician’s office and then provide it to us.
