Upload Your Imaging Order
Upload your script to ensure you’re scheduled for the correct exam and avoid delays in your care.
If your scheduled exam does not match the order from your provider, your appointment will need to be rescheduled. Reasons for this include:
- The authorization from your insurance carrier may not match
- An available time for the correct exam may not be available
- Ensure appropriate prep (ex: no food or drink after midnight) has been communicated and followed
Without an order in our system from your provider, our schedulers can only interpret the information provided at the time of scheduling and make the best determination from that information.
After submitting the form, you should receive a confirmation email from “Smartsheet” confirming that your submission was successfully sent.
How to Upload Your Imaging Order
If accessing from your computer:
- Save your script to your computer
- Upload the script from the saved location
From your phone:
- Take a picture of your script
- Upload the script from your photos
If the exam on your order DOES NOT MATCH the exam you are scheduled for, a member of our team will contact you to reschedule your appointment for the correct exam.
If the exam on your order matches the exam that you are scheduled for, no additional action is necessary, and you will not be contacted.
